It’s time to evaluate this year’s Museums and Galleries Month. Our evaluation forms are now available; the deadline is 11 June but the sooner you return your form (after May) the better.

You can use one of two forms. One is our online survey form - just click here to link to the form and press submit when you are done.

Alternatively you can use our Word format form. Click here to open it and save. You can fill it in, save and email it back to us at info@campaignformuseums.org.uk, or you can print it out and fax it on 020 7233 6770 or you can put in the post – address on the form. If you would like us to send you a preprinted form to fill in, just email as above.

Please let us have your responses by Wednesday 11 June.

Don’t forget our MGM Commendation Scheme: there are three £500 awards to be won. Just download the Word format entry form here and email back your response with pictures. The MGM Working Group will judge the responses.
